Insurance
The Indiana Eye Clinic participates in many insurance plans including Medicare and most commercial plans. If you have questions about your specific coverage, or concerns about billing, please contact our insurance department. Since most insurance companies consider Refractive Surgery (Laser Vision Correction) an elective surgical procedure, it is usually not covered; however, some plans offer their members a discount. Please check your coverage or contact one of our insurance coordinators to check on your specific plan.
Patient Forms
To expedite your first office visit, please help us by completing the Patient Information and Medical History forms before your appointment, and returning them by email to us. You may also print and complete the forms and bring them to your appointment along with your current medical insurance information. Established patients should update their forms annually.
