Our experienced doctors, nurses, and staff are committed to providing clearer vision and complete optical health to every patient -- whether you need a comprehensive eye exam or suffer from nearsightedness, farsightedness, astigmatism, aging vision, or special health related vision problems such as cataracts, glaucoma, macular degeneration, diabetic eye disease, or other medical eye conditions. Insurance
The Indiana Eye Clinic participates in many insurance plans including Medicare and most commercial plans. If you have questions about your specific coverage, or concerns about billing, please contact our insurance department. Since most insurance companies consider Refractive Surgery (Laser Vision Correction) an elective surgical procedure, it is usually not covered; however, some plans offer their members a discount. Please check your coverage or contact one of our insurance coordinators to check on your specific plan.
Patient Financing
Often times insurance does not cover all necessary healthcare expenses. This can put a financial strain on those who need medical procedures such as eye surgery. One option is a solution offered by Forum® Credit Union: a “MoneyLine” line of credit.
Visit www.forumcu.com for competitive interest rates, payment calculator, and loan application. Click on personal / auto loans. Other options, such as a Health Savings Account, may be a good choice for you. Please ask your bank or finance company what options may be available.

Patient Forms
To expedite your first office visit, please help us by completing the Patient Information and Medical History forms before your appointment, by printing and completing the forms. Once completed bringing them to your appointment along with your current medical insurance information. Established patients should update their forms annually.

 If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Adobe® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.
No Show Policy
Please note that a $25 fee may be assessed for missing a scheduled appointment without contacting us to cancel or reschedule at least 24 hours in advance. Please let us know immediately if you are unable to honor your time commitment to your doctor.
